COVID-19 Pandemic Program Acknowledgement and Disclosure
I understand that during this COVID-19 Pandemic the procedures listed in this disclosure statement are for the safety of all persons present at the facility and to limit everyone’s risk of exposure to the extent reasonably possible.
I understand that I and/or my children are highly encouraged to use hand sanitizer and are required to wear a mask. I and/or my children must practice proper social distancing and remain six (6) feet from all other people, except for family members per the Thurston County Health Department guidance.
I also understand that myself, and my children must be free from COVID-19 symptoms. Therefore, me and my children will be asked if we have any of the below symptoms upon entering the facility. You and/or your children need to be symptom free, in accordance with the latest DOH guidance. As of June 2020, if you or your children has COVID-19 symptoms, they must remain in isolation until ten days after the symptoms started, and three days after the fever is gone (without use of medications), and after other symptoms have improved before returning. If any of the following symptoms appear during a program, myself, or my child will be asked to leave as soon as possible.
- Fever of 100.4 degrees Fahrenheit or higher
- New loss of Taste or Smell
- Dry Cough
- Sore Throat
- Shortness of Breath or Difficulty Breathing
- Muscle or body
- Chills - Rash
- Abdominal Pain
- Nausea, Vomiting or Diarrhea
- I or my child has been in close contact with someone within the past two weeks who is currently sick with suspected or confirmed COVID-19.
- I or my child has been diagnosed or tested positive for COVID-19 within the past two weeks.
While we understand that many of these symptoms can also be related to non-COVID-19 related issues we must proceed with an abundance of caution during this Pandemic Emergency. These symptoms typically appear 2-7 days after being infected so please take them seriously.
I further understand, while I and my children attend this program, we will be in contact with other children, families, and volunteers who are also at potential risk of exposure by the community. I understand no list of restrictions, guidelines or practices will remove 100% of the risk of exposure to COVID-19 to myself or my children as the virus can be transmitted by persons who are asymptomatic and before some people show signs of infection. I understand that we play a crucial role in keeping everyone at the program safe and reducing the risk of exposure by following the practices outlined herein.
We will work with the local public health department and DOH to identify any or all close contacts who will need to be quarantined. I understand that this may also include providing information (names, phone number, addresses) of participants if there is an exposure per Department of Health (DOH) regulations. Puget Sound Anglers will also follow the guidelines and recommendations of DOH and the local health department to determine the duration of the closure of the program/facility and to determine other steps that should be taken to limit the spread of the virus.
After reading the foregoing information, I certify that I have read, understand, and agree to comply with the provisions listed above. I acknowledge that failure to act in accordance with the provision listed or with any other procedure outlined will result in termination of event.
General Program Risk Waiver
I am fully aware that there are inherent risks, including but not limited to physical injury, to which I or my minor child will be exposed to because of the nature of the activity for which I have registered. I assume said risks and agree to hold harmless South Sound Chapter Puget Sound Anglers and their assigns from any damages resulting therefrom.
(A parent/guardian will be asked to sign this form while checking in at this event.)
It is with great pleasure that the South Sound Chapter Puget Sound Anglers announces the return of our annual Kids Fish-in at Longs Pond in Lacey, WA on Saturday April 17, 2021 from 9:00 AM to 12:00 PM. This event offers kids 4 to 14 years of age an opportunity to learn how to fish, be safe on the water and finally to catch a fish. Each participant will receive a fishing pole and terminal tackle at no cost.
We plan to place a net along 400’ of the shoreline of Longs Pond planted with 2,000 rainbow trout provided by the Washington Department of Fish and Wildlife. The netted area will be marked with 8’ spacing providing up to 50 participants an opportunity to try out their new fishing pole. Each participant will be allowed to fish for one hour or until they catch two fish within the netted area. The netted area will reopen at 1:00 PM for anyone who wants to take another try at catching a trout. There will be volunteers to provide technical advice and to clean the fish for successful anglers.
In addition to the fishing opportunity, we will offer instruction and demonstrations on boater safety with an actual boat on premises, how to cast and how to identify the various fish species in our area.
All participants must pre-register with the South Sound Chapter Puget Sound Anglers prior to April 15, 2021 using either of the links below. Participants will be scheduled at hourly intervals beginning at 9:00 AM to avoid overcrowding.
